Why was Plains Trail Group Home Created?

PTGH was created to welcome adults with intellectual and or developmental disabilities (IDDs). Residents will live together, benefit from the support of a caring team, and enjoy the comradery with other adults with IDDs in an engaging residential and social program.

Does PTGH require pre-admission Assessments? 

Every potential resident will be asked to stay at PTGH during a "Guest Stay" for three to seven days.  This time shall be used to assess if PTGH is the right fit for your loved one.  We are unable to guarantee admission after the visitation period.  We will reimburse 80% of the Administration fee if admission is not possible at PTGH.

Who lives at Plains Trail Group Home?

Residents of PTGH are individuals with IDDs who love to have fun, enjoy socializing with peers and are actively able to engage with our staff.  Residents must want to live at PTGH.  No one will ever be required to stay. Residents are adults of at least 18 years of age who have intellectual disabilities, developmental disabilities, or autism.  


Residents may need substantial levels of support, including medication monitoring, physical assistance, and help with daily logistics, personal care, and individualized scheduling.  
 

Cost:  How much does it cost to live at PTGH?

Monthly fee depends on the private accommodations assigned to the resident (see below).  Most residents will pay for our services by trusts or private pay. Traditional insurance policies or government programs (Medicare and Medicaid) will not cover our services or living costs.  We ask that you work with your agency representatives to coordinate medical care or medications that can be administered or scheduled at PTGH.

One Time Administration fee: $1000 due when we schedule the "Guest Stay"

Private Room: $5400 per month  (3 rooms total)

Semi-Private Room (two individuals in the room):  $4300 per month (1 room total)

What is included in My Residency? 

 

 Staff and Activities

  • Staffing 24 hours a day (awake nights) / 7 days per week / year-round

  • Sparrows Austin Daily Activity participation Monday through Friday 10am to 3pm and special events.

  • Participation in group and individual scheduled activities including community outings.

  • Supplies for planned activities and events

  • Birthday and other personal celebrations including supplies

 

 Meals and Personal Needs

  • Meals every three hours during the day:  breakfast, lunch, snack, dinner.

  • Meal preparation, assist with cooking and encouragement during assigned routines.

  • One restaurant outing per week as a group outing.

  • Toiletries and other: bathing and toileting supplies, select additional supplies.

  • Linens, towels, and laundry room supplies.

 Accommodations

  • Private or Semi-Private Room with furniture, linens, internet, use of common areas and facility supplies

 Supportive Services

  • Personalized program based on individual plan focused on health and quality of life.  

  • Medication monitoring and facilitation (no nursing care)

  • Pickup medications from pharmacy and medication storage.

  • Health support:  communication with doctors and communication with guardians.

  • Transportation (must have at least one week notice:  doctors’ appointments, events, activities).

  • Support daily hygiene and personal care as needed based on individual plan.

  • Regular communication with family members and guardians.

What is not included?  

  • Nursing care or medical services.

  • External professional healthcare services: These must be secured by guardians, but can be coordinated at PTGH.

  • Additional restaurant outings not part of the Sparrows Austin outings or restaurant outings organized by PTGH.

  • Sporting, concert, or other types of events not coordinated by Sparrows Austin or by PTGH intended for all residents.

Does PTGH accept insurance or government assistance? 

No. Residents will pay for residency by private pay or by trusts.  

Medication and medical co-pays must be paid for by guardians.

Do you provide in-home medical services?

No. PTGH is not a medical facility or nursing home.  We do not have nurses or specialized staff.  Medical services must be coordinated by the guardians, communicated to PTGH via the resident service plan and PTGH may coordinate medical visits (outside of the center or at the facility).  We are able to monitor doctor directed medical plans, make appointments, and coordinate community services at the facility.
